As they explore, eerie phenomena ghostly apparitions, shifting corridors, and fragmented memories that blur the lines between past and present. Sammy’s psyche fractures into multiple versions of herself — a frightened child, a vengeful teen, a silent observer, and a terrifying, otherworldly entity known as the Hollow. Mandy must confront each fractured self to reclaim her friend.
Escaping the asylum doesn’t bring relief. The Hollow latches onto them, bleeding into their reflections, dreams, and reality itself. A cult called the Children of the Hollow emerges from the shadows, intent on awakening the full power of the Hollow and using Sammy as a key to tear open the veil between worlds.
With the city unraveling around them and their identities blurring, Mandy chooses to enter Sammy’s fractured mind. There, she battles through haunting memories and manifestations of trauma, finally confronting the Hollow’s core — a fractured mirror born of grief and madness. By embracing forgiveness and strength, Mandy shatters the Hollow’s hold, freeing Sammy and breaking the cycle of torment.
Though the asylum’s curse is broken, subtle changes linger — reflections that move on their own, whispers in the dark — suggesting the Hollow’s echo endures. Mandy and Sammy survive, forever changed, walking a world that is never quite the same again.
